A scheduled review of all the Assessment Framework documentation was undertaken during late 2011. There was extensive consultation with HACC service providers, Commonwealth Respite and Carelink Centres, Regional Assessment Services, ACATs and other key stakeholders which resulted in changes being made to the suite of documentation.
The independent evaluators GGJ Consultants recommended a review of the current WA Assessment Framework documentation to ensure information is presented in a clear and succinct manner and duplication of data entry is minimised.
The new streamlined HACC Eligibility and Client Assessment Form combines the CNI, Assessment form, a revised Occupational Safety and Health risk screen, the HACC fees screening form and the support plan documentation.
The Regional Assessment Services - Interagency Communication Plan has been removed as information about the client receiving assistance from more than one service provider is captured on the new HACC Eligibility and Client Assessment Form.
Service providers will receive the following documentation from the Regional Assessment Services for each client/carer:
