HACC Quality reporting rounds happen once every three years for HACC funded service providers. As part of this process we visit you and conduct a Quality Review and we will notify you in writing ahead of time when your first visit is due.

Number of visits - each HACC funded agency will usually receive one Quality visit within each three-year cycle. You will be required to submit an improvement plan after our team’s initial visit and then a minimum of one each year afterwards.

Extra visits - if your agency has difficulty meeting the standards we may plan further visits and improvement plan reviews to help you.

Joint visits – if you operate both Commonwealth and HACC funded programs from the same site/outlet you will receive a ‘joint visit’. Both a representative from CommunityWest and the Department of Health and Ageing will attend.

HACC only service delivery site – visited by a CommunityWest Quality Review team. 

Commonwealth only service delivery site – visited by a Department of Health and Ageing review team.
