For example, the revised Aged Care Assessment Team (ACAT) professional development workshop, Goal Setting: Assessment and Documentation, continues to be delivered to ACATs, Hospital Social Workers and Discharge Coordinators (HSWs) in public and private hospitals.

After six months post-implementation of the 'Health, Wellness and You' project 108 Centre Based Day Care clients from the six service providers involved have participated in the Stanford Model of CDSM (chronic disease self-management) workshops. The feedback provided via the service providers’ reports has been positive, although, the implementation of the workshops has not come without its challenges.
