The work of CommunityWest extends beyond the Home and Community Care (HACC) sector into many related and linked sectors. For example, the revised Aged Care Assessment Team (ACAT) professional development workshop, Goal Setting: Assessment and Documentation, continues to be delivered to ACATs, Hospital Social Workers and Discharge Coordinators (HSWs) in public and private hospitals.

This workshop is the amalgamation of Being a HACC Coordinator and the WAAF and Implementing Client Goals and Support Plans workshops. This course is for WA HACC coordinators working in WAAF regions only.

Sector Development has commenced delivering the much anticipated Wellness in Action in CBDC for Managers and Coordinators workshops in the metropolitan area. Have you reserved your place yet?

After six months post-implementation of the 'Health, Wellness and You' project 108 Centre Based Day Care clients from the six service providers involved have participated in the Stanford Model of CDSM (chronic disease self-management) workshops. The feedback provided via the service providers’ reports has been positive, although, the implementation of the workshops has not come without its challenges.
