HACC Quality Reporting 2008 – 2011 Round Follow Up Visits

The next three year cycle has commenced with the Multi Purpose Sites (MPS). We have received most of the self assessments and are planning our visits which will take place over the next twelve months.

For our other providers, at the conclusion of your follow up visit you will be advised if you require any further support or if you are being referred to the next reporting round – being the 2008 – 2011 round. Those referred to the new round will be scheduled for a full Quality Reporting process within the coming three years.

 
HACC Quality Reporting 2005 – 2008 Round Follow Up Visits

The follow up visits with agencies have commenced and the team is delighted that the majority of agencies visited to date have commencing implementing, if not completed implementing their agreed actions and our collecting evidence to support demonstration of new processes.

Agencies who have not been contacted yet for a follow up visit will be contacted 6 months after your initial visit. For rural and remote sites, you will be contacted once we know we are visiting your region.

 
HACC Quality Reporting 2005 – 2008 Round

There were 138 HACC service providers identified from the HACC database that were required to participate in the 2005 – 2008 Quality Reporting round. This round did not include the Multi Purpose Services (MPS) and individual sites

From the 138 identified sites for visits, 123 visits were completed.

There were 162 recommendations given to agencies. The following table captures the results of the completed 123 site visits and the types of recommendations made (Agreed Actions) by standard and outcome:

Summary of Recommendations

CommunityWest and the Department of Health/HACC are discussing identified trends and issues and developing a plan of action to assist agencies to meet these standards and outcomes. Ideas such as resources and training/workshops are all being investigated.
